Please bring all pertinent X-rays, MRI's, CT Scan's, and EMG reports to your appointment. Also please have any relevant office notes and previous operative reports with pictures forwarded from your previous physician's office. This information will allow our doctor to best treat you. Also please bring a list of your medications and the doses of each for your record.
